The Company
Work for a reputable, community-based not for profit organisation that provides choices for people with disability, supporting them to regain and maintain their independence within an inclusive community. The role can be based in any of our office locations where fleet vehicles are available, allowing for convenient access from your home.
The Role
This role is central to ensuring people can access the right care at the right time. You'll be the first point of contact for prospective clients, combining a strong customer focus, sales and advisory skills to guide them through the intake process and support a smooth transition into home care services.
Your day to day duties will include but not limited to:
- Attend clients houses in response to enquiries for prospective support at home
- Conduct in-home assessments and preliminary client sign ups across Victoria
- Guide clients and their representatives through home care options, costs and person centered plans
- Identify and support equipment and home modification requirements for clients
- Manage end-to-end intake and onboarding, including service agreements, documentation, and activation of services
- Review the client portal for referrals and coordinate timely follow-up and engagement
- Maintain accurate records across and contribute to continuous improvement initiatives
What will you need to succeed?
- Demonstrated knowledge of Support at Home and/or Home Care Package programs
- Experience working in an allied health / NDIS / community based role is desirable but not essential
- A valid police check and valid driver's license, and someone who is comfortable to drive as part of your role
- A positive driven individual who has a willingness to learn
- Exceptional customer service skills with the ability to show empathy and compassion
